The Federal Poverty Guidelines are used in creating and annually updating the sliding fee schedule (SFS) to determine eligibility. Please contact our office for more information on how to complete a sliding scale application. We accept Medicaid, Medicare, BPA Funding, private insurance as well as private payments.
